Immunization Requirements 2010-2011
Below is the summary of the Papillion-La Vista School District School immunizations rules and regulations. The PLSD follows the Nebraska Immunization Program guidelines set by the Nebraska Department of Health and Human Services.
A copy of each student’s immunization record must be presented before beginning the enrollment process.
Students enrolling in a toddler/pre-school program must have received 4 doses of DtaP, DTP or DT vaccine: 3 doses of Polio vaccine: 1 dose of MMR given on or after 12 months of age: 3 doses of Hib vaccine or 1 dose of Hib given at or after 15 months of age: 3 doses of Hepatitis B vaccine: and one dose of varicella (chicken pox vaccine) given on or after 12 months of age. Students enrolling in kindergarten must have received 3 doses of DTaP, DTP, DT or Td vaccine, with one shot given on or after the 4th birthday: 3 doses of Polio vaccine: 2 doses of MMR vaccine, given on or after 12 months of age and separated by at least one month: 3 doses of Hepatitis B vaccine: and one dose of varicella (chicken pox vaccine) given on or after 12 months of age. Students enrolling in seventh grade and all transfer students from outside the state of Nebraska must have received 4 doses of DtaP, DTP, DT or Td vaccine, with one shot given on or after the 4th birthday and 1 dose of TdaP on or after 10th birthday: 3 doses of Polio vaccine: 2 doses of MMR vaccine given on or after 12 months of age and separated by at least one month: 3 doses of Hepatitis B vaccine: and one dose of varicella (chicken pox vaccine) given on or after 12 months of age and prior to 13 years of age. If over 13 years of age, two doses of varicella are required, separated by at least one month. School PhysicalsNebraska law requires that all students entering kindergarten or seventh grade have a physical exam prior to the start of the school year. Exams completed more than six months prior to the first day of school will not be accepted. Physical exams are also required for students transferring into any grade level from schools outside of the state. Parents who do not want their child to receive a physical may sign a waiver, available at each school. A copy of the school physical forms is available by clicking here.
