Below is the summary of the Papillion-La Vista School District School immunizations rules and regulations.  The PLSD follows the Nebraska Immunization Program guidelines set by the Nebraska Department of Health and Human Services.

A copy of each student’s immunization record must be presented before beginning the enrollment process.

Students enrolling in a toddler/pre-school program must have received 4 doses of DtaP, DTP or DT vaccine: 3 doses of Polio vaccine: 1 dose of MMR or MMRV given on or after 12 months of age: 3 doses of Hib vaccine or 1 dose of Hib given at or after 15 months of age: 3 doses of Hepatitis B vaccine: 4 doses of pneumococcal or 1 dose of pneumococcal given on or after 15 months of age: and one dose of varicella (chicken pox vaccine) given on or after 12 months of age. Written documentation (including year) of varicella disease from parent, guardian, or health care provider will be accepted. Click here to see the preschool parent letter.

Students enrolling in kindergarten must have received 3 doses of DTaP, DTP, DT or Td vaccine, with one shot given on or after the 4th birthday: 3 doses of Polio vaccine: 2 doses of MMR or MMRV vaccine, given on or after 12 months of age and separated by at least one month: 3 doses of Hepatitis B vaccine: and two doses of varicella (chicken pox vaccine) given on or after 12 months of age. Written documentation (including year) of varicella disease from parent, guardian, or health care provider will be accepted. Click here for the kindergarten parent letter. 

Students enrolling in seventh grade and all transfer students from outside the state of Nebraska must have received 3 doses of DtaP, DTP, DT or Td vaccine, with one shot given on or after the 4th birthday and 1 dose of TdaP on or after 10th birthday: 3 doses of Polio vaccine: 2 doses of MMR or MMRV vaccine given on or after 12 months of age: 3 doses of Hepatitis B vaccineor 2 doses of adolescent vaccine if student is 11-15 years of age: and two dose of varicella (chicken pox vaccine) given on or after 12 months of age. Written documentation (including year) of varicella disease from parent, guardian, or health care provider will be accepted.  Click here for the 7th grade parent letter.
